One of the images shows an ad for Phoneix insurance. Is this true? Does this book have ad(s)?

I don't think that is an ad, I think it is showing the " Important Papers" pockets. That just happens to be the papers I the pocket!

How many pages are in this book?

21 pages back to back with a pocket page for papers at the end.

Why does one cost $12.39, but two costs $78.00?

It's a different seller. If you use the drop-down to change the quantity the price should stay the same.
