Does the adapter work with 18v tools?

No only the 36v multivolt tools. So if it says 18 V and multivolt it still doesn’t matter. It Has to say 36 V multivolt

The Metabo HPT MultiVolt Adapter can only be used on Metabo HPT 36V tools. The accessory will not work on another brand's tools. Hope this helps!
