Would you use this on a roof? Are the friction pads conducive to sticking to roofing shingles?

Use the ProVisionTools PiViT Laddertool for that. It's great on stairs too and works as a ladder scaffold, but costs $100.The Levelizer is a bit of a toy compared to the Pivit, but may be more convenient to quickly stick under a ladder on a dirt slope.

Will it work for ground sloped in two directions? My yard slopes away from the house but also slopes left to right.

Yes ditto to other answers, diagonally worked for me just fine. Of course I don't know how extreme an angle you're talking about.At first i thought I was crazy for buying it, but after I used it a bit, I concluded it was well worth the bucks I spent on it, especially compared to getting hurt by falling.

No idea if it is OSHA approved. If you go to provisontools.com and look at Ladder Helper, this device (more expensive) might be OSHA approved.
