Where does the thin copper heatsink get placed? I don’t see it in the pictures and there is no manual or instructions I could find.

The two finned heatsinks go onto the two chips on the top of the board and the copper one goes onto the one chip on the bottom of the board, you'll see it's size corresponds to the size of the copper heatsink.

Does fan run continuously or is it engaged only when cooling is needed?

It's continuously running at all times when it is power is engaged

Will this be compatible with the new Raspberry Pi 4?

Yes. I have a Rpi4 in a NesPi case. It cooled my Raspberry 60~65c to between 45c and 50c. It came with to sets of thermal pads. One marked for the pi4 and the other set is for the pi3B+/3B. This fan is not a screw on fan.

Why are two pieces of thermal tape includes for the 3B+, when the directions say to choose either or?

the same tape is included for different models that require the additional tape.
