Do the instructions advise using only tap water or can distilled water be used?

You can use distilled, filtered, or tap water. Distilled is best because there will be less buildup, especially if you have hard water. (Don't use tap water with water softener in it.)

is this a cold diffuser and not a heat diffuser as I know that heat destroys some of the essential oil components?

I've had a couple of these for about a year and while initially, they did not heat up the water, they now do. The water is quite warm which is exactly what you don't want with essential oils. I wouldn't buy it again.

Is it also an air purifier

No. But it is also an ionizer. The diffuser doesn't heat up and it puts negatively charged ions into the air and helps with bacteria and moods etc. There isn't a filter. I really like this for a beginner diffuser, helped my household stop coughing at night and i like to put couple drops of thieves essential oil and man my home smells good! Hope this helps

How does one clean this diffuser?

It is very easy to clean. Just rinse with warm water and I wipe it dry with a paper towel or napkin. It is ready to be used again.
