Yes, you can use those with Form 4. In the ORI space put the following: WVATF0800 ATF-NATL TRA CTR Martinsburg, WV In the Reason Fingerprinted put: NFA Application Form 4

These forms are approved background check fingerprints card. These are used industry wide in law enforcement and CWP classes. FBI also use the same forms. Yes they are acceptable for form 4.

These cards are the exact replica of the US Government FBI cards that all state agencies use. I'm not sure about what code you are referencing, but they are the FD-258 Fingerprint cards. I use them by the 100's every month. i send them to all of the state agencies, and the FBI without a single problem.

This type of card is acceptable for any Law Enforcement requirement. They also apply to Federal as well. They are the standard for almost every agency in the US.
