Q: description says ddr4, features says ddr3. which is it?

If you purchase from PC Server and parts, you will receive unit as title states. i5-6500 w/ 16GB DDR4 RAM. We have submitted a request with Amazon to correct errors.

Q: This item states wireless wifi (adapter). does that mean the wifi in not harware installed in the desktop? assuming means a usb wifi adapter (specs)?

The unit I received in October 2022 came with a bluetooth adapter, no wifi adapter

I will do my best to answer your question, but the way the question is phrased leads me to believe you want to install a 2nd hard drive, meaning leaving the original in place. If that's the case, the answer is no. You can only have 1 x 3.5 hard drive. You might be able to put 2x 2.5 hard drives in there. However, the computer is designed to take at least 1x 3.5 hard drive. You can use one 2TB 3.5 or 2TB 2.5 hard drive.
